Supported Independent Living (SIL)

Conze Disability Support Services provides assistance with daily tasks within shared or individual living environments to support participants to live as independently as possible.

Our focus is on stability, structured routines, and practical daily support aligned to each participant’s NDIS plan.

Service coverage

York and surrounding Wheatbelt communities and Perth metropolitan area
Subject to capacity and suitability assessment.

Supports included

• Assistance with personal care

• Meal preparation and household routines

• Medication prompting (as per support plan)

• Support with daily living activities

• Skill development within the home

• Support with community participation

All supports are delivered in accordance with the NDIS Practice Standards and within our approved registration scope.
